Canadian Immigration Medical
You are required to book an immigration medical with a Canadian authorised physician within the UK. You can find a list of these here (for England, you can search for other countries too).
We used Nuffield Health in Manchester (we have family there, so combined the visit!). I just called the clinic and booked the appointment. They required our passport number and which type of medical we needed. Pricing for the medical varies depending on location and if you require any extra tests. But the general consensus is around £300 per person. So because there's two of us, we had to pay a lovely £600! Yet again, it's totally worth it when you get that email saying you've got in!

You're required to take your passport, and eye glasses/contact lenses and any relevant medical history paperwork if you have any health issues.
Our doctor was lovely and very reassuring. From what I can remember the urine test was to test for diabetes and the blood test for HIV (I think). Our medical didn't even last an hour, so you're in and out pretty quickly. The doctor isn't responsible for giving you any results or telling you if you "pass" the medical. I think they only contact you if they find anything that you were medically unaware of. They uploaded the results to a central database that the Canadian government can access, this is usually done pretty quickly (ours was about 1-2 working days). They also gave us a report that we scanned and sent to our lawyer which had info on how to access the database.
